Event Planning Forms
All forms should be passed in to VP Activities & Events (SUB 408)
Event Planning Forms - Due by 5:00 on Tuesday of the week prior to your event.
Example - if you are planning an event for Friday, September 17, these 3 forms are due by 5:00pm on Tuesday, September 7.
Student Risk Assessment - The planning process starts with this form. Anyone wishing to hold an event on campus must fill out the Student Event Risk Assessment Form. All approval signatures need to go on this form. Once this form has been completed,
Event Budgeting - This form is meant to help you guide and maintain the financial aspects of your event.
Liquor License - This form is necessary for any event that plans on having bar services.
Post-Event Forms - Due within 48 hours of the end of the event.
Post Event Evaluation - This form is mandatory for any event organizer to fill out after the event has taken place. Once this form has been completed, submit it via email to Mark Gillis - VP Activities & Events at su_activ@stfx.ca
